Wait time is one of the clearest ways a patient experiences a health system. You do not need a dashboard to understand what three months feels like when you are waiting for an answer.

But operationally, “the wait is too long” is a description of the outcome. It is not yet a diagnosis of the system.

The 2026 national data matters — and it is still the first layer.

CIHI’s June 2026 release shows a mixed picture across Canada: several priority procedures are approaching or returning to pre-pandemic performance, while many cancer-surgery waits remain longer and diagnostic-imaging delays persist. CIHI also frames emergency-department waiting as a symptom of pressure across the broader system.

A wait-time number tells us where pain is visible. It does not tell an operator which lever to pull.

One wait can have several causes.

A long diagnostic wait could reflect a real shortage of scanners, technologists or interpreting physicians. It could also reflect incomplete requisitions, conservative scheduling rules, unused off-hour capacity, no-shows, weak cancellation recovery, uneven demand across sites or a referral queue that has not been segmented properly.

Those are different problems. Buying capacity is expensive. Redistributing capacity is operational. Improving referral quality is a workflow intervention. Central intake is a routing intervention. If we call all of them “a wait-time problem,” we risk paying for the wrong fix.

I would want to see five clocks, not one.

Referral-to-triage. Triage-to-booking. Booking-to-test. Test-to-report. Report-to-clinical follow-up. Then I would look at slot utilization, cancellations, no-shows, requisition quality, priority mix and how often work crosses organizational boundaries.

Access depends on capacity and on how well the system matches the right patient to the right capacity at the right time.
